Nominations wanted for "Love Your Heart!" Award
  
The Toronto Heart Health Partnership is accepting nominations for the fourth
annual "Love Your Heart!" Award. The award recognizes people who have made
healthier food choices, become smoke-free, become physically active or helped
others make healthy lifestyle changes.

"The award recognizes changes like increasing the number of daily servings of
fruits and vegetables, making walks a part of your lifestyle, quitting smoking,
or declaring your car or home smoke-free," said Jane Forest, a member of the
award organizing committee.

Heart disease is the number one cause of death in Ontario for both women and
men.

All residents of Toronto are eligible to apply and self-nominations will also
be accepted. Recipients will be invited to receive their award at a special
reception in June hosted by the Toronto Heart Health Partnership.

The Toronto Heart Health Partnership includes public health, community health
centres, businesses, volunteer organizations, recreation organizations, boards
of education and hospitals that work together for heart health.

Call Toronto Public Health at 416-338-7600 for nomination forms or for more
information. Nomination forms are also available in libraries, community
centres or online at www.toronto.ca/health. The nomination
deadline is April 10, 2005.
